RESOLUTION NO. R-6-2008

A RESOLUTION OF THE BOARD OF COUNCIL OF THE CITY OF FORT THOMAS, CAMPBELL COUNTY, KENTUCKY, AUTHORIZING MAYOR MARY H. BROWN TO EXECUTE AN AGREEMENT BETWEEN THE CITY AND HUMANA PERTAINING TO THE PROVISION OF HEALTH, DENTAL, LIFE AND VISION INSURANCE FOR THE 2009 CALENDAR YEAR.

WHEREAS, the city has determined it is beneficial to enter into an agreement with Humana to provide health insurance, dental insurance, life insurance and vision insurance to employees.

WHEREAS, quotes were solicited for the provision of health insurance, dental insurance, life insurance and vision insurance for employees.

NOW, THEREFORE, BE IT RESOLVED, by the Board of Council of the City of Fort Thomas, Campbell County, Kentucky:

That the Mayor be, and hereby is, authorized and directed to execute an agreement with Humana to provide the following insurance benefits to employees:

Health Insurance:  Kentucky National POS 100/70 Copay Plan

Dental Insurance:  Kentucky PPO 1000 MAF 100/90/60

Life Insurance:  Basic Employee Life and AD&D

Vision Insurance:  Exam Plus EP
